- Education: Bachelor’s degree relevant to the job
- Training: Eight (8) hours of relevant training

Requirements

- Recently updated Notarized or Sworn Personnal Data Sheet (CSC Form 212 with passport size recent photo)
- Work Experience Sheet (Attachment to CSC Form 212)
- Transcript of Records and/or Certificate of Units Taken for Unfinished Degrees/Courses
- Certificate of CSC Eligibility/Valid Professional License
- Certificate of Participation or Completion of trainings/seminars attended
- Service Record for Gov’t employees (Outside DA- PRRI), Certificate of Employment (For non-DA- PRRI Personnel)
- IPCR/OPCR for June – December 2023 with at least a Very Satisfactory rating (For government employees)
- Certified photocopy of Special Order for designations in managerial & supervisory positions (for applicants to division chief levels) [If any]
